3 phase meters are "normal" ANSI sized.. For high voltage and high 
amperage locations they have current transformers (induction coils)
that factor what the meter sees. ie: a standard meter  (US ANSI)
is 200 amps. Put some 2000:1 CT's on it and it's a 400kamp meter.

Note: A US Ansi meter is a big round can, even though the modern 
electronics for a 200 amp meter fit in something the size of a circuit 
breaker.
