You can get a reasonable estimate by looking at a pre-terminated fiber for that distance.  Add lineitems for termination boxes and  labor  1-2 days labor for 2 techs.  (If you email me I'll tell you the vendor I use for pre-terminated fiber, but searching should yield results).
